I have a Payza Personal account I hadn't used Payza much for cashouts before. I thought that the 5% fees when cashing out by Payza meant we didn't have to pay any more fees. Buch out of my $100 cashout, 5% was held back here and another 2.5% was held by Payza. Is this really how it's supposed to work?

For starter Payza accounts there's no additional charge, but for personal/business accounts there's fee of 2.5%+0.25$
In your case, 95-2.375-0.25=92.375$

Ouch!
I just assumed it was kind of like Neobux and Clixsense (before the whole US-thing), where we pay the fees when cashing out and don't have to pay any more afterwards.
But I still get that extra fee, why are the Twickerz fees so high?

Because when twickerz received money from members twickerz received high fees also
Quote: Payza
* Starter accounts pay no receiving fees on up to $400 USD or equivalent/month or $2,000 USD.
** Foreign exchange rate includes 2.5% over the wholesale exchange rate at which we buy our currencies. Exchange rates are adjusted regularly.
Note: Higher fees apply to some industries: 3.90 % + $0.59 USD for receiving funds.
